Patient Resources

Before your first appointment, we require the information listed below to support your care.

Once your appointment is scheduled with AZSports Medicine in collaboration with Honor Health, you will have access to MyChart, HonorHealth’s secure patient portal. Most required forms can be completed electronically through MyChart prior to your visit. We encourage you to complete all available forms in advance.

Please be prepared to provide the following information, if applicable:

  • New patient registration information
  • Authorization for release of medical records
  • Insurance card(s)
  • Workers’ compensation or PIP information
  • Referral documentation, if required by your insurance plan
  • Imaging related to your current condition (X-rays, if applicable)
  • Current medication list
  • Co-payment, if applicable
  • Any forms not completed through MyChart
  • A parent or legal guardian must accompany patients under 18 years of age
